Who are we?

About Us

The Health and Wellbeing Partnership works to improve health, wellbeing and social care services for the people of Lambeth.

Formerly known as the Health and Social Care Partnership, we include representatives from Lambeth Adults' and Community Services, Lambeth Primary Care Trust, hospitals that are used by Lambeth residents and voluntary sector organisations.

We support Lambeth First on health and social care issues, and contribute to the wider work of the Lambeth First partnership to promote the economic, social and environmental wellbeing of Lambeth.  By working together we have increased opportunities to share information, plan services and target our work more effectively.

In partnership with health providers in Lambeth, we aim to tackle both the influences on health and improve the health and wellbeing of people with longer-term health problems and disabilities.


Our Vision

The goal of the Health and Wellbeing Partnership is to make a major contribution to Lambeth First’s Sustainable Community Strategy commitment to improve the health and wellbeing of residents, which will enable them to live active and independent lives.

In achieving this we will create active, healthy and independent communities, closing the gap the standard of health within the borough and between the borough and the rest of London. 

Health services within Lambeth will also be equally accessible to all members of our community. Health and social care services will be based on enabling independence, providing choice and promoting control.

We will achieve this by giving patients more personalised care and greater choice around the health care services they receive.
