Transitions pathway

There is a transitions group at Lambeth Council made up of senior care managers and commissioners from Children and Young People's Services and Adults' and Community Services.

They have been working to make a clear transitions pathway, or plan, to help young people and their families through transitions. This pathway is important as it will help every young person going through transitions to have the same opportunities.
