Transitions

a teenage girlTransitions is what social services call the time when a young person goes from Children and Young People's Services to Adults' and Community Services. Transitions starts when a person is 14 years old or in year 9 at school. It will finish when they are 18 or 19 years old.

Transitions planning is an important part of the transitions process. It is when you sit down with your family, your teachers, your care manager, your GP or health professional and plan the next few years.
