Health action plan

Here at Lambeth we have health action plans which are a bit like health passports. A health action plan lists what you can do to be healthy. It helps you get the health services you need.

It is good to show your health action plan to doctors, dentists, nurses or anyone who helps with your health care.

If you do not have a health action plan you can download one here. You may need some help filling it out so do ask your family, carer or whoever you want to help you with it.
