Referral Form and Criteria

To access services available through Young & Safe the young person must meet the required target group.  To check whether a young person meets the criteria please refer to the ‘Young & Safe Criteria Protocol' document below. 

Before submitting a referral you should notify the young person/ family/ carer that you are making this referral.

The lead professional should fully complete the referral form below and email it to youngandsafe@lambeth.gov.uk

Safeguarding Protocol

Please find below the Safeguarding Protocol which Young & Safe have agreed with Lambeth Social Care in relation to gangs. This document outlines where Young & Safe will act as a lead agency and where Social Care will be involved and the processes that other professionals should follow.
